Get thumbnail/cover image for a book

GET 
/api/v1/books/:book_id/thumbnail

Extracts the first page and resizes it to a thumbnail (max 400px width/height). Supports HTTP conditional caching with ETag and Last-Modified headers, returning 304 Not Modified when the client has a valid cached copy.
